Per ES type (AUDIO_ES, VIDEO_ES, SPU_ES) is used by interfaces (like: qt4 for menu items). However from the meta data this order cannot be related to any select video, audio or spu because this information is not present in the meta data. By adding this meta data item as Index one can find out which meta data information is related to the selected audio, video, spu track as presented by interfaces.

By using playlist item the metadata information can also be queried when the item is not playing. When using current input this can only be done when the input is instatiated (aka: playing).

This removes deprecated argument from libmpeg2 functions.
Only mpeg2_internal.h remains used and needs to be updated,
but it can also be installed as a system header.

This also fixes warnings in xcommon.c.

Previously, this vout didn't work for me. (nVidia)
It is aimed to be tested on Intel/Via XvMC capable hardware.
