This new callback is triggered when an element is added/removed from
the list, or when the list is cleared.

Also remove the filter chain buffer functions update hack, keep constant
callbacks for video filters in the filter chain, remove now useless
parameters from filter_chain_New(), and inline
filter_chain_AppendFilterInternal().

This will end up useful for cubemap implementation and may allow to put mms stuff out of the core

Also remove a useless and unsynchronized test.

This really only depends on the input item(s) and the core.

ASF allows specifying a new aspect ratio while playing back.
Triggering change using the "aspect-ratio" variable can't work
for unknown A/R as the variable is "Checked" type.

Those are more flexible than the GNU-specific %m format specifier to
print standard error messages. They are also less likely to format a
clobbered error number.

This mostly reverts commit 8e3dd711.

Remove static data, and keep each callback correctly scoped to its
LibVLC instance.

(Functions reordered to avoid forward declaration.)

Those interfaces that need playlist deactivation can now call it
explicitly.

Contrary to input_GetAout(), this works even without input thread.
