ide: add DECLARE_GENERIC_PCI_DEV() macro to generic IDE PCI host driver

* Add DECLARE_GENERIC_PCI_DEV() macro to generic IDE PCI host driver
  and use it to cleanup generic_chipsets[].

* Remove dead/obsolete code while at it.
