* all : begin to rewrite some parts of avi demux, mainly to clean ugly code

and to prepare for OpenDML support.
Add new options :
  --avi-index force index creation.
  --avi-interleaved force method used for unseekable stream but need
interleaved stream and could lead to loss of audio/video synchro.
